I'm not totally sure, but most game companies do have stuff about defective copies, and exchanging them for a different one. Not sure about Fallout though, and even then it may not be inclusive to those in the UK. Might be worth a shot before going out and buying another copy though.
Ok, here is another Idea, try turning off hardware accelaeration on your soundcard. I assume a Soundblaster. I know when you get into the elevator a sound file loads up. You might see if that does the trick.
It's weird how Fallouts crash in some occasions without any logical explanation. Like in Fallout2 when I sleep with Miria or Davin. Or when entering Military Base in Fallout. The screen goes black for about a minute then jumps back to windows. Sometimes I get through.
